Population Health Approach to Neck Pain Neck pain and its associated disorders (NPAD) cause significant health burden in the general population and after road traffic and occupational injury. Individual-level health care treatments have been well studied, but population-health approaches to this problem have not. The study being referenced used a best-evidence synthesis to examine population-level approaches to the prevention and control of NPAD. A study in the July 2008 issue of the Journal of Manipulative and Physiological Therapeutics looked at a systematic review examined studies published between 1980 and 2006 that addressed the incidence, prevalence, risk factors, prevention, cost, assessment and classification, interventions, and course and prognostic factors for NPAD. Citations were screened for relevance, scientifically reviewed, and synthesized. Valid studies addressing public policies or population-level approaches to the prevention and control of NPAD were identified and used in the evidence synthesis. The study concluded that there is little evidence on which to make public or population-level recommendations, despite the important public health burden and costs of NPAD. Population-level approaches to preventing and controlling NPAD should be investigated. Effects of Spinal Manipulative Therapy in Children Younger Than 3 Years A study in the July 2008 issue of the Journal of Manipulative and Physiological Therapeutics was published to identify any adverse effects to chiropractic care occurring in the pediatric patient and to evaluate the risk of complications arising in the pediatric patient resulting from chiropractic care. The study reviewed a 3-year retrospective study of pediatric case files from the Anglo-European College of Chiropractic (AECC) (Bournemouth, England) teaching clinic practice in Bournemouth, England. Most (73.5%) patients presenting were 12 weeks of age or younger. This study shows that for the population studied, chiropractic manipulation produced very few adverse effects and was a safe form of therapy in the treatment of patients in this age group. X-Rays
Effective for Colon Cancer An X-ray test may help doctors determine who needs regular colonoscopies. A federal study published in the New England Journal of Medicine performed both standard colonoscopy and X-ray tests on more than 2,531 people at 15 U.S. medical centers. The study found that "virtual colonoscopies" identified 9 out of 10 people who had cancers or large growths seen by regular colonoscopies. However, the study also found that technicians could misread the X-ray and identify polyps that weren't there, leading to unnecessary follow-up tests. It also was not as good as standard colonoscopy at detecting flat growths on the colon wall, which are more likely to be cancerous than more prominent growths, according to the Associated Press. The researchers say the study was more effective at ruling out cancer than finding it. to Radiography Guidelines for Low Back Pain and Chiropractic Schools A study in the July 2008 issue of the Journal of Manipulative and Physiological Therapeutics was published to describe instruction provided at chiropractic schools worldwide on the use of spine radiography and compares instruction with evidence-based guidelines for low back pain. Individuals responsible for radiology instruction at accredited chiropractic schools throughout the world were contacted and invited to participate in a Web-based survey. The survey included questions on the role of conventional radiography in chiropractic practice and instruction given to students for its use in patients with acute low back pain. This survey suggests that many aspects of radiology instruction provided by accredited chiropractic schools appear to be evidence based. However, there appears to be a disparity between some schools and existing evidence with respect to the role of radiography for patients with acute low back pain without "red flags" for serious disease. This may contribute to chiropractic overutilization of radiography for low back pain.
