Survey: Employers want wellness programs

Three-quarters (72.5 percent) of employers have or want to have a wellness program that utilizes a health-risk assessment, according to a survey released by United Benefit Advisors LLC (UBA), a benefits-advisory organization. The survey polled 1,664 employers.

“The tide has clearly turned in that the majority of employers believe in the long-term value and positive impact that comprehensive wellness and disease management programs provide,” said William Stafford, vice president of member services for UBA. “Keeping the healthy employees healthy while stabilizing and/or improving the health of employees with chronic conditions will dramatically improve everyone’s health and quality of life.”

Other clear trends emerged related to employers’ perspective on what the government’s role should be in improving the healthcare landscape. Just 26 percent of employers want the government to develop a taxpayer-funded universal healthcare system. Without regard to employee size or geographic region, more than 75 percent of employers surveyed felt the government should require hospitals and doctors to:

  • Disclose pricing;

  • Require insurers to disclose actual discounted prices paid to providers; and

  • Mandate healthcare provider quality reporting.

The survey also indicated that more than 98 percent of respondents were confident they would provide healthcare benefits to their employees one year from now.

Food additives and hyperactivity

There is evidence reported in the British Medical Journal, that supports a trial period of eliminating colorings and preservatives from the diet.  Whether preservatives and colorings cause or exacerbate hyperactive behaviors is an important question for many pediatricians and parents.  A recent randomized placebo controlled trial in 297 children aged 3-9 years provides evidence of increased hyperactive behavior after they ate a mixture of food colorings and a preservative (sodium benzoate).

In contrast to many previous studies, the children were from the general population and did not have attention-deficit/hyperactivity disorder. The trial found an adverse effect of the mixture on behavior as measured by a global hyperactivity aggregate score. The daily dose approximated that found in two 56 g bags of sweets.

In view of the potential importance of these findings, the European Food Safety Authority (EFSA) recently provided an opinion that takes other evidence into account.

More baseball players return to pre-injury levels after elbow vs. shoulder surgery

An article in the online newsletter, OrthoSuperSite discussed how investigators discovered that when comparing injuries to the elbow vs. injuries to the shoulder, 45% of pro athletes re-entered at the same or higher playing level.

Investigators found that more professional baseball players return to the same or higher playing level after elbow surgery than they do following shoulder procedures and that higher-level players tend to not reach their pre-injury performance levels than do some minor league players.

In a retrospective review, Steven B. Cohen, MD, and colleagues studied the records of 44 professional baseball players who underwent surgery for shoulder or elbow injuries and assessed their return to baseball. The investigators discovered that 45% of the all professional baseball players, including A-level minor league players, returned to the same or higher level of professional baseball after surgery and found that 52% of players having elbow surgery returned to that level compared to 35% who had shoulder surgery.

The study also revealed that 39% of players retired after surgery and that only 22% of upper-level professional, including major league and AAA and AA minor league, players were able to return to their pre-injury levels.

Cohen and his colleagues reviewed the records of AAA, AA, A and major league baseball players in one professional club during a four-season period from 2003 to 2006. The study included 26 players who underwent shoulder surgery (28 procedures) and 21 players who had elbow surgery (23 procedures). Three players underwent both procedures. There were 19 pitchers in the shoulder group and 20 in the elbow group.

“At the high professional level — major league baseball, the AAA and AA level — only one of 12 players was able to return to that high level,” according to one of the study’s authors. Of the patients who had labral repairs, 10 retired from baseball after surgery, six returned to the same or higher playing level, five went to a lower level and one remains in rehab.

In the elbow group, 12 players underwent ulnar collateral ligament reconstructions, seven underwent loose body excision, three had ulnar nerve transpositions, and one had a flexor debridement. Overall, investigators found that 11 of 21 players returned to the same or higher playing level after surgery.

House passes directive on chiropractic care

The U.S. House of Representatives has approved a directive that orders the Pentagon to make chiropractic care a standard benefit for all active-duty military personnel. 

The legislation is contained in H.R. 5658, a bill authorizing defense programs in fiscal year 2009, and is based in part on recommendations from the American Chiropractic Association (ACA) and the Association of Chiropractic Colleges (ACC).

The bill — passed by the Armed Services Committee on May 14 and the full House on May 22 — also contains language allowing for chiropractic demonstration projects at overseas military locations and clarifies that chiropractic care at U.S. military facilities is to be performed only by a doctor of chiropractic.

In 2000, Congress passed and the president signed a similar authorization bill, which contained language calling on the secretary of defense to develop a plan to get the chiropractic benefit to all active-duty service members
