Physician groups ask Bush administration to postpone NPI deadline

According to the AMA News web site, Starting “May 23, 2008, physicians must use only their National Provider Identifiers (NPIs) in place of any other IDs when filing an electronic claims and some paper claims with Medicare and other payers”.

Already extended by a year by the Bush administration, some organizations representing doctors urge CMS to add an additional six months to the May 23 deadline. Allowing physicians to use both the legacy and NPI numbers will help to ensure a smooth transition without disruption of claims processing and payment.

A Psychophysical Study of Auditory and Pressure Sensitivity in Patients With Fibromyalgia and Healthy Controls  

A study in the May 2008 issue of The Journal of Pain looked at pressure sensitivity in patients with Fibromyalgia.

Fibromyalgia (FM) is characterized by widespread tenderness. Studies have also reported that persons with FM are sensitive to other stimuli, such as auditory tones. The authors hypothesized that subjects with FM would display greater sensitivity to both pressure and auditory tones and report greater sensitivity to sounds encountered in daily activities.

These findings from the study support that FM is associated with a global central nervous system augmentation in sensory processing. Further research is needed to examine the neural substrates associated with this abnormality and its role in the etiology and maintenance of FM.

The perspective of the study found that muscle tenderness is the hallmark of FM, but the findings of this study and others suggest that persons with FM display sensitivity to a number of sensory stimuli. These findings suggest that FM is associated with a global central nervous system augmentation of sensory information.

These findings may also help to explain why persons with FM display a number of comorbid physical symptoms other than pain.

HMOs block key brand-name medicines to make big bucks, report says

According to a recent NY Daily News article, HMOs are blocking patients from getting critical single-source drugs - like Lipitor and Celebrex - to boost company profits.

Single-source drugs are brand-name products with a unique chemical form that are sold by one manufacturer and have no less expensive generic equivalent.

And the HMOs are either charging patients top dollar in co-pays - up to $100 - or forcing doctors into time-consuming fights over the prescriptions approved, says the report, to be released Sunday by state Sen. Jeffrey Klein (D-Bronx, Westchester).

Sometimes the drugs aren't covered at all, according to the report, which surveyed how and whether the 15 HMOs in the state with drug plans restrict prescriptions of 20 common single-source drugs.

Aetna, Oxford, Cigna, HIP, GHI and Health Net of New York were among the HMOs surveyed, and all were accused of putting up obstacles, in varying degrees, to obtaining single-source medications.

The report says drugs with restrictions include those for high cholesterol, like Vytorin and Crestor, medications for depression and anxiety, like Lexapro, prescriptions for diabetes, such as Actos and Avandia, and anti-asthmatics, like Singulair and Advair Diskus.

The state's HMO trade association immediately condemned Klein's report as "simplistic and broad-brushed hyperbole that demonizes the HMO industry."

The Mediating Role of Depression and Negative Partner Responses in Chronic Low Back Pain and Relationship Satisfaction

A study in the May 2008 issue of The Journal of Pain looked at the mediating role of depression in chronic low back pain.

Chronic low back pain (CLBP) is a prevalent pain condition associated with increased disability, lower quality of life, and poor relationship satisfaction. However, little research has examined the impact of the psychosocial environment in predicting relationship satisfaction among persons with CLBP.

This study examined empirically supported psychosocial variables as potential mediators in the association between pain and relationship satisfaction. Patients with CLBP completed depression, partner support, pain, relationship satisfaction, pain catastrophizing, and pain-related fear measures (N = 54).

Negative responses by a partner and depression were found to mediate the association between pain and relationship satisfaction, with negative responses emerging as the most important mediator.

The current findings are consistent with a biopsychosocial framework of chronic pain and suggest that negative interpersonal interactions in patients with CLBP may be of central importance when considering psychosocial intervention. Theoretical and practical implications for treatment are discussed.

Perspective – This study suggests that psychosocial variables, specifically depression and perceived negative partner responses, have a significant impact on relationship satisfaction among individuals with CLBP. These findings highlight issues integral to the social adjustment of patients with CLBP.
